Continuity 

The scar on Utah's face disappears when he leaves the group for a day while they are in the chalet mansion after the party.

First time you see Pappas and Utah in the VW there are mismatching license plates attached to the car: a German license plate with WI for Wiesbaden at the front and a French plate (as in the rest of the movie) at the rear.

In the last scene, Utah is snowboarding. At the top of the run he has no helmet, once riding down the slope he is wearing a helmet.
at the first ordeal, there are 3 persons, at the second (and third), there are 2 persons, at the fifth ordeal, there are 3 persons and Utah, at the sixth ordeal, there are 4 persons and Utah, Clearly all males. From then on, the number decreases correctly, according to each death.

28 minutes in the movie, after the night out on the yacht, Pappas is driving Utah around in his SUV (grey VW Touareg).

The shot in town and in the the tunnel shows the front of the car with a German license plate "WI UX 955".

When they arrive at the building, the car has a French registered license plate "QM 645 LC".

At the final chase scene where Utah catches up with Bodhi at the train station and they stare at each other from across the tracks, Bodhi has windblown hair hanging down in his face, but a subsequent cut back to him shows his hair is perfectly combed back, looking perfectly groomed.

In the scene after Johnny and Bodhi crash their dirtbike's and johnny is talking with Hall and Pappas, Pappas lights a cigarette, then 10 seconds later lights another one.

Errors in geography 

In the last scene, Utah is snowboarding in the Wasatch Mountains, Utah. There are glaciers in the surrounding mountains, there are no glaciers in Utah.
In the beginning of the movie, when he goes to find people surfing, out "in the middle of the ocean", one camera shot reveals a quick glimpse of the beach being very close right behind them.

Tofana is a cable car that starts from the town of Cortina D'Ampezzo and stops at the top of a tall rocky mountain. There is no town or village at the top of the Tofana cable car, not even buildings other than that of the cable car.

Angel falls pools at its base then has a number of cascading rock pools rather than a flat river. The water level is quite low in the approach shots in the canoe making it unlikely there would be sufficient water flow to push a body over the edges of each rock pool then a mile down to the flat where he wakes up.

Factual errors 

Angel Falls is 3,212 feet (980 m) tall. Jumping from the top of the falls would kill a person. Due to the height of the fall a person would reach terminal velocity (130 miles per hour/209 kilometres per hour). The chance of surviving would be minuscule as at those speeds water won't compress. It would be like hitting a brick wall at 130mph/209kmph. You might be able to survive by pointing your toes and going straight in, but you would break every bone in your feet, legs, hips, and probably your back as it compresses.
Venezuela does have an extradition treaty and the FBI would know that.

The mountain explosion and subsequent bank robbery are apparently set in Italy and all of the Carabinieri (local military police) is speaking Italian, yet almost all vehicles have foreign registration plates.

Miscellaneous 

When Utah jumps off Angel Falls to chase Bohdi for the 8th ordeal and lands at the bottom of the falls alongside him, contrary to a goof listed, this would not kill him, because there is no water tension. The thing that would've killed him would have been the shallowness of the falls or drowning from the rapid currents. It is unlikely that someone would survive such a feat, but it is possible, and is done in the movie.

Incorrectly regarded as goofs 

When Bodhi attacks the bank branch, Utah gets a gun and pulls back the slide before reinserting the magazine. He is actually performing a 'brass check'. Pulling back the slide enough to ensure a round is chambered before reinserting the magazine. A standard tactical practical.

Plot holes 

When the cash is cut loose from the pallets after being dropped out of the plane the skydivers would have entered the cave of the swallows long before the cash hit the ground. The surface area of the dollar bills would have slowed the fall. Only solid bundles of cash would have reached ground level at a similar time,

Revealing mistakes 

The hugely mountainous waves towards the end of the film hardly rocked the small boat nor did they make the two characters lose their balance while they were on deck talking to each other.
In the opening scene, Johnny's partner tells him their line is un-ridable, yet when they do ride it, there are many tracks of people who rode it before.

During the first heli-assisted snowboarding descent numerous tracks appear be carved in the snow during close up shots of the riders. It is implied that this descent has never been attempted before.

Factual errors 

The helicopter that carries Utah out to confront Bodhi at the film's end is a UH-1 which has a maximum range of 315 miles. When Utah is speaking to Bodhi he says that he is 150 miles offshore. With the severe winds and rain the helicopter's maximum range would be severely curtailed...no pilot would agree to fly in such conditions.
